Patient Information
Thank you for your interest in Desert Spine and Sports Physicians. Please note that we do not overbook so that we can devote the necessary amount of time to completely evaluate and treat each patient. Therefore please give us as much notice as possible if you are unable to make your scheduled appointment. This will allow us to schedule someone off of our waiting list. While we realize that unexpected events do occur, if you no-show or cancel your visit less than 24 hours in advance, you may be responsible for a cancellation fee. We look forward to seeing you at Desert Spine and Sports Physicians.
How to Prepare for Your First Appointment:
We use Phreesia, a secure online check-in system, to help make your visit faster and easier. After your appointment is scheduled, you will receive a text message and email with a link to Phreesia. This link will allow you to:
- Verify your identity
- Enter your demographic information from your own device (smartphone, desktop, or tablet)
- Complete consent forms
- Upload a photo of your ID and insurance card (this helps us verify insurance eligibility before your visit)
We ask that you complete this process before arriving at the office.
For your first appointment at Desert Spine and Sports Physicians, please bring three completed forms:
- New Patient Form – download, fill out, and bring with you.
- Notice and Acknowledgement of Privacy Practices – read the Notice of Privacy Practices, then download, complete, and bring the Acknowledgement of Privacy Practices form with you.
- Authorization Signature Form – download, complete, and bring with you.
To request copies of your medical records, please fax or drop off this Medical Record Release form to any of our locations. You may also request/download copies of your records via our secure patient portal, Healow. After registering, simply send us a message including the destination information. Please note, for security purposes we are unable to email your records. You are welcome to pick up during normal business hours.
If you are unable to complete the online check-in, paper forms are available in the office. Please plan to arrive 30 minutes early for consult appointments, and 15 minutes early for follow-up appointments, as we still require a separate medical history intake form that is not yet available in Phreesia.
- Please bring the following items with you on your first visit:
- Related radiographic CD’s or films and reports (i.e. X-ray, MRI, CT)
- List of current medications with dosages
- Valid insurance card and driver’s license
- Referral from your primary care physician (if required)
- Payment (co-pay) if applicable – we accept cash, check, and credit card
How to Prepare for Your Follow-up Appointment:
When you come to your follow-up appointment(s) with Desert Spine and Sports Physicians, you are encouraged to download and bring the completed Follow-up form to your visit. If you are unable to download our forms, please plan to arrive 15 minutes prior to your appointment in order to fill out the follow-up patient form which is available at our front desk.
How to Prepare for your Electrodiagostic Study (Nerve Conduction Study and EMG):
