Ganglion Impar Block for Chronic Tailbone Pain

Calendar May 29, 2026

Tailbone pain is easy to dismiss—until it isn’t. Sitting through a workday, driving across town, or simply rising from a chair can become genuinely difficult when the coccyx is the source of persistent, unrelenting discomfort. For patients who have tried conservative measures without meaningful improvement, a ganglion impar block may offer a path forward that doesn’t involve surgery or long-term medication.

At Desert Spine and Sports Physicians, our fellowship-trained physiatrists and pain management physicians evaluate and treat complex pelvic and spinal pain conditions. Here’s what you need to know about this targeted, minimally invasive procedure.

What Is the Ganglion Impar?

The ganglion impar is a small cluster of sympathetic nerve tissue located at the base of the spine, just in front of the coccyx. It serves as a relay point for pain signals arising from pelvic and perineal structures—including the coccyx as well as the rectum, bladder, urethra, and surrounding tissues.

When this ganglion becomes sensitized or overactive, it can amplify and perpetuate chronic pain, including tailbone pain, in ways that are difficult to address through conventional approaches alone.

What Conditions Does a Ganglion Impar Block Treat?

A ganglion impar block is used to treat chronic pain in the tailbone. Conditions that may respond to this treatment include:

  • Coccydynia (chronic tailbone pain)
  • Perineal pain
  • Pelvic pain with a sympathetically mediated component
  • Post-surgical or post-traumatic pelvic pain
  • Rectal or anal pain without a structural cause
  • Chronic pelvic pain that has not responded to other interventions

Because the symptoms associated with ganglion impar dysfunction can overlap with other conditions, such as hemorrhoids, nerve entrapment, and others, a thorough evaluation is essential before proceeding with any intervention.

How Does the Ganglion Impar Block Procedure Work?

The procedure is performed by one of our physicians using fluoroscopic guidance—real-time X-ray imaging that allows precise needle placement near the ganglion impar. A local anesthetic is injected to interrupt pain signals traveling through the ganglion temporarily.

The block typically takes less than 15 minutes to complete. Patients are monitored briefly before discharge and may return to light activity the same day. As with all procedures at Desert Spine and Sports Physicians, this procedure is performed exclusively by a physician to ensure the highest standards of safety and accuracy.

What Results Can Patients Expect?

For most patients, there’s noticeable pain relief within just days of their procedure. For some, relief is sustained; for others, a series of injections provides the most durable benefit. This finding helps guide further treatment decisions.

Individual outcomes vary depending on the nature and duration of the underlying condition, and our physicians are committed to setting realistic expectations through transparent, honest communication.

Is a Ganglion Impar Block Right for You?

A ganglion impar block may be appropriate if you have been living with chronic tailbone pain that has not responded adequately to physical therapy, anti-inflammatory medications, or other conservative treatments. It may also be a reasonable next step if prior imaging or evaluations have ruled out other diagnoses.

The best way to determine whether this procedure is appropriate for your specific situation is a comprehensive consultation with one of our physicians, who will review your history, imaging, and treatment trajectory before making a recommendation.
